HEALTH INFORMATION
The school should be aware of any serious illness that your child may have had and any prescribed medication your child may be taking; in some cases this medication must be kept in the school office. The day's dosage must be sealed, labeled and have the pupil's name attached with the prescription from the doctor; it is to be kept in an appropriate container in the school office. NO medication is to be kept in the classroom.
The student is solely responsible to know when to come to the office for the medication. The student will not be called since classes cannot be interrupted. Please notify the school office of these situations.
The school follows the state law, therefore, aspirin and other over-the-counter drugs will NOT be dispensed to students.
Parents MUST provide the school with original updated medical and immunization records. (HRS Form 680 or Form 680, Part A and HRS-H Form 3040). Forms must be updated every two years.
Guidelines for the State of Florida include:
- A second dose of measles vaccine (preferably MMR) for grades Kindergarten through fourth.
- Children entering, attending or transferring to seventh grade will be required to have completed the hepatitis B vaccination series, a second measles vaccination (preferably MMR) and a tetanus-diphtheria booster.
- Florida Certification of Immunization, Form 680 was revised to incorporate the new immunization requirements.
- County health departments who are on the Health Clinic Management System have the capability to generate a computerized Florida Certification of Immunization.
Scoliosis exams are given to all 8th grade students. If your child
receives a notice recommending further examination, the completed form
must be returned to the school office.
If any student becomes too ill to remain in class, he/she should report
to the office, once they have received the teacher's permission. If
it is necessary for the student to be sent home, the parent will be called
by the school office staff.
EMERGENCY CARDS
Every parent is asked to complete an emergency card annually. These cards are kept on file and will be used as the need arises. If you change your address or telephone number, business or residential, please notify the school receptionist, as it is necessary that we have the correct information on your child's card. It is important that we are able to reach you easily, especially in an emergency. In the event of sickness or injury, the parents or other person as indicated on the Emergency Card will be called. It is extremely important to name someone who can pick-up and care for your sick child. The school will not be responsible for any student, if parents do not provide the Administration with up to date information on emergency contacts.
EMERGENCY PROCEDURES, TROPICAL STORMS, HURRICANES
In the event of hurricane warnings or other emergencies, Saint Brendan
Catholic Elementary School will dismiss and resume classes in accordance
to public announcements for Archdiocesan Parochial Schools. Parents
will follow radio or television announcements in this regard.
If emergencies or tropical storms arise during school hours, parents or authorized persons are urged to pick up their child or children immediately at school. NO student should remain on school grounds when these emergencies arise.
